Position Overview
The Integrated Health Consultant is responsible for developing, evaluating, supporting, and/or expanding integrated health programs and initiatives for the Iowa PCA, and to promote a culture of process improvement. The Integrated Health Consultant will develop and maintain community and state partnerships and increase awareness of integrated health as an integral component of overall health. The successful candidate in this position will serve as the subject matter expert (SME) for oral health services. Focus areas for integrated health consultants include, but are not limited to, medical, oral health, behavioral (brain) health, supportive services, vision, pharmacy, and other health and wellness service areas as needed and identified. Consultants are expected to work across all focus areas and continually increase their knowledge and understanding of all services. Integrated health is a coordinated, team-based, person-centered approach, designed to bring together all components that make humans healthy - physical, oral, pharmacy, and behavioral (brain)- focusing on providing comprehensive care to improve outcomes for all.
Key Responsibilities
Serve as a leading contributor for the Iowa PCA’s integrated health strategy work in collaboration with the Integrated Health team and other staff as identified. This position is responsible for providing ongoing opportunities for health center staff to connect around integrated health work, creating technical assistance and training opportunities for health centers, and continued blending of integrated health as outlined in the Iowa PCA Strategic Plan and IowaHealth+ Roadmap.
Serve as the lead subject matter expert for the assigned focus area(s) including providing technical assistance (TA) and training to participating health centers, coordinating activities with participating health centers, cross-state PCA partners, and HRSA, ensuring all grant objectives are met, participating in all required calls and meetings, and assisting with the creation of progress reports and other documentation as needed.
Application of continuous improvement and facilitation tools when providing training and technical assistance to health centers, creating measurable and sustainable improvements in patient and health center outcomes.
Manage assigned grant program initiatives including coordination of participating health centers and the grantor, ensuring that all grant objectives are met, participating in all required calls and meetings, and creating progress reports and other documentation as needed.
Facilitate Integrated Health meetings, Work Groups, Committees, Learning Cohorts, and facilitate/participate in statewide and cross-state Coalitions to discuss innovations and ongoing strategic planning related to Integrated Health and assigned focus areas.
Participate in the development of integrated health strategies/solutions, training, and technical assistance for alternative modalities of care within the health centers. These may include, but are not limited to, eHealth/telemedicine, home and community health visits, care management, school-based clinics, and mobile health.
Assist health centers in maximizing the use of their Electronic Health Record (EHR) to support workflows and data collection to support projects and other network goals and initiatives. Develop subject matter expertise for the hosted EHRs related to assigned focus area(s), if applicable.
Work cross collaboratively with internal teams to provide content, support, and guidance to Iowa PCA, IowaHealth+ and INConcertCare committees and work groups related to Integrated Health and assigned content for specific focus area(s).
Qualifications/Experience
Bachelor’s degree required. Candidates with a combination of professional clinical licensure and experience will be considered.
License to practice the person’s specific profession in the State of Iowa preferred, if applicable.
5+ years of clinical, public health, or administrative/managerial experience in oral health related field. Experience in direct patient care is required.
Administrative, management, performance improvement, quality assurance, and grant writing experience preferred.
Preference given to candidates with experience in Federally Qualified Health Centers (FQHCs) and/or Rural Health Centers (RHCs) or direct experience with Medicaid population.
3+ years of experience in program management preferred.
1+ years of experience managing process improvement and change initiatives for health care teams or in other settings - demonstrated ability to motivate and encourage successful process changes in ambulatory care settings and delivering measurable improvements is strongly preferred.
Knowledge of public health principles, billing, and coding and practices preferred.
Excellent oral & written communication skills; able to communicate effectively with organization leadership, health center staff, clinicians, community leaders, and government officials.
Experience planning and facilitating meetings and group work in a highly collaborative, interdependent environment.
Experience using data analysis to drive initiatives and/or process redesign in either a consulting role or a supporting role, preferably in healthcare.
Must be flexible, self-motivated and a team-player.
Ability to travel around the State of Iowa at least 25% of the time to foster relationships with health center staff through in-person contact.
